Health Project was awarded the Trinity Health Innovation Award at the Trinity Health Clinical Conference October 25th, for the Innovation pilot to integrate their IT system, ClarkeIS, into Great Lakes Health Connect to share social determinants of health information with clinical providers.

In the spring of 2015, Health Project worked with Trinity Health IT and Etherios to develop a program to track patients through Health Project, looking at their social determinants of health. ClarkeIS has been in use for more than two years and has helped to better connect patients to health and human service agencies and assure their clinical needs are met.
