Health Project Hub Healthy Future
A Health Project Hub program designed specifically for youth ages 12-18. This program assesses youth for health risk factors and then pairs the youth and their family with a Community Health Worker (CHW) to help them make healthy lifestyle changes to enter adulthood healthy and happy.
