Community Health Needs Assessment
The Community Health Needs Assessment (CHNA) is an in-depth report that engages community-based organizations, health care institutions, and community members to identify and prioritize the current needs of our community. The Health Project facilitates multiple focus groups and distributes several surveys to ensure we’re doing all we can to listen, understand, and support initiatives that improve the communities in which we serve.
Community Impact Grants
The Community Benefit Board Initiative and the Sister Simone Courtade Fund were created to support, strengthen, and sustain Trinity Health’s organizational mission in the community. The programs promote optimal health for poor and vulnerable populations by addressing social needs, connecting social and clinical care, and reducing health inequities.

The Health Project connects to the community in more ways than you may realize. It’s important to all of us that our community has the opportunity to be the best it can be.
