Community Health Needs Assessment

The Community Health Needs Assessment (CHNA) is an in-depth report that engages community-based organizations, health care institutions, and community members to identify and prioritize the current needs of our community. The Health Project facilitates multiple focus groups and distributes several surveys to ensure we’re doing all we can to listen, understand, and support initiatives that improve the communities in which we serve.

Community Impact Grants

The Community Benefit Board Initiative and the Sister Simone Courtade Fund were created to support, strengthen, and sustain Trinity Health’s organizational mission in the community. The programs promote optimal health for poor and vulnerable populations by addressing social needs, connecting social and clinical care, and reducing health inequities.

As our services move forward into better serving our West Michigan neighbors, we are also streamlining the support we offer our community partners. Please continue to visit this page for updated Community Impact Grants program announcements, key dates, and engagement opportunities.

For application information and/or questions regarding the 2025 award cycle, please reach out to Program Implementation Coordinator, Michael Ramsey.

News & Events

The Health Project connects to the community in more ways than you may realize. It’s important to all of us that our community has the opportunity to be the best it can be. We’re always looking for more ways to be involved in various area events, especially those that concern the overall health and well-being of Muskegon and Oceana counties. If there’s an event or opportunity that you’d like to share, send an email with your contact information and we’ll reach out for more details.
