Health Project Hub of the Lakeshore

The Health Project Hub of the Lakeshore program is designed to help clients with complex or urgent medical needs by pairing them with a Community Health Worker to assist them in navigating the medical system; as well as helping to provide for their social, emotional and spiritual needs. The Community Health Worker will connect the client with needed services, while teaching them how to successfully manage their own care and chronic conditions.  Want to learn more?
