We pride ourselves on the ability to serve patients of the Lakeshore with access to care issues through the use of our single-enrollment application initiative.
Getting started is easy:
- Complete an application, (English) or application (Spanish) (print the application, complete it and bring it into the office M-F between 9:00-4:00 with proof of income and ID. Any questions, call 231-672-3201.)
- Review the list of required documentation to make sure you bring everything we will need to help you.
- Turn in your application during walk-in hours or make an appointment. MCHP staff will work with you to help you enroll in programs for which you or your family may be eligible.
